Lead agency ONEgeneration, partnering with Alzheimer?s Association Southern California Chapter (AA-CS), Northridge Dignity Hospital, California State University Northridge, and key stakeholders, will improve the quality of respite and home-based services and coordination of care through the expansion of services to the Greater San Fernando Valley, Santa Clarita Valley, and Antelope Valley. Services will include home-based case management to Alzheimer?s Disease and Related Dementia?s (ADRD) individuals, adult day care for individuals with intellectual or developmental disabilities (IDD), and community-based education, outreach, and support to caregivers and workers caring for ADRD individuals.
